This woman in her 50s had lost the fullness and clean double eyelid of her youth. she underwent upper blepharoplasty with filler to the lower lids and here she is at the one year follow up. the double crease on the inner corners and hanging skin on the outers have been removed giving her an even lid platform across the full length of her lids. the restylane smoothed the contour between her lower lids and cheeks. she was delighted and showed me a photo from her 20s with nearly identical appearing eyes. so gratifying!

This woman in her late 60s felt she had lost the brightness of her big eyes and now looked more startled. upper blepharoplasty was performed. the ellipse of skin removed was widest in the center and greater on the left side in order to create an even lid platform on both sides. she was very pleased with her softened expression.
This woman in her 50s was unhappy with her tired looking eyes. she underwent upper blepharoplasty surgery and is seen at 3 weeks. her eyes are now more open and she has a visible lid platform where she can now apply eyeshadow. her incisions are still pink but this will fade and disappear over the next month or so. she is thrilled with her results and loves hearing how great she looks.
I end up doing a lot of revisions on upper eyelid surgeries performed by other surgeons. most commonly the excess skin has been removed in the shape of a crescent moon resulting in maximal skin excision centrally leaving too much excess skin in the inner and outer corners. this patient was unhappy with the new “rounded” shape of her eyes. in the top photo the skin to be removed has been outlined. the shape is more of a “dumbbell” as seen in the middle photo. the least amount of skin is removed centrally. at the completion of surgery (bottom photo) the new incision line has created a fold following the arch of the lashes creating a more almond shape. i look forward to seeing her back and sharing her healed results!
This young woman was self conscious of her drooping left upper eyelid. ptosis surgery has many fine nuances and that’s why i love it so much. both the height and shape of the lids have to match. in the photo below after ptosis repair, the level and contour of the lids are much more symmetric. results like this are very gratifying and make me appreciate being an oculoplastic specialist.
Our eyes play such an important role in our facial expression that small amounts of aging can have a big impact. this 36 yo woman was becoming self conscious of looking angry and appearing to be in a bad mood. in the photo above, the excess skin in the upper lids is worse in the center, causing a flatness of the eyelid fold. three months after an upper blepharoplasty, photo below, the arch of the upper lid fold now follows the arch of the eyelashes creating a brightness of the eyes and a happy expression.
Many in-office consultations involve dark circles, and whether filler or surgery would be best. this 42 yo woman presented with concerns of dark circles and tired looking eyes. in her photo above, the nasal and central f*t pads are bulging and project beyond the lid/cheek junction below. filler in this case would only emphasize the bags so surgery was recommended. the photo below is taken one month after lower blepharoplasty. while there is still some mild swelling and pinkness of the incisions, the under eye transition with the cheek is smoother leading to a more well rested appearance.
This 53 yo presented with aged, tired appearing eyes. while upper blepharoplasty usually focuses on skin and the inner f*t pad only, in this case the marked puffiness required more aggressive f*t removal across the full length of the upper lid. similarly in the lower lids, f*t contouring was combined with removal of excess skin. the external approach, used here, allowed excision of redundant skin from the lower incision edge. in the photo below, taken at 3 months, the upper lids are more open and brighter, and the lower lids have a healthy youthful appearance. even with considerable skin removal, the lower lids maintain their natural shape.
